This worksheet directs employers to examine the workplace environment
and what food choices and information are available in the workplace.
The object of this worksheet is to encourage employers to assess
how well they currently serve the needs of their employees with
diabetes and to offer guidance on how to improve.

This worksheet is designed to assist employees with diabetes better
understand how their particular health plan covers services relating

Employers should fill out the answers to the questions on this
worksheet before providing it to employees. If more than one health
plan option exists, employers should provide employees with the
completed information for all health plan options so that employees
can make an informed choice.
